北京2025年4月25日 /美通社/ -- 强生公司今日宣布，旗下创新治疗药物锐珂®（埃万妥单抗注射液）正式获得国家药品监督管理局批准，与卡铂和培美曲塞联合给药，适用于治疗携带表皮生长因子受体（egfr）19号外显子缺失或21号外显子l858r置换突变且在egfr酪氨酸激酶抑制剂治疗期间或之后疾病进展的局部晚期或转移性非鳞状非小细胞肺癌（nsclc）成人患者[1]。此次获批标志着埃万妥单抗在中国迎来了今年第二个肺癌适应症。此前，锐珂®已于今年2月获批用于治疗携带egfr 20号外显子插入突变的局部晚期或转移性非小细胞肺癌成人患者的一线治疗[1]。
此次获批是基于mariposa-2 iii期研究强有力的有效性和安全性数据。研究结果表明，与单独化疗组相比，埃万妥单抗联合化疗可将疾病进展或死亡风险降低52%，中位无进展生存期（pfs）为6.3个月，而单独化疗组为4.2个月（hr=0.48；95%ci：0.36–0.64；p<0.001）[1]。研究同时观察到，mariposa-2 亚洲亚组人群中，与单独化疗相比，研究者评估中位pfs为10.3个月，而单独化疗组为4.2个月（hr=0.39；95%ci：0.26–0.59；p<0.0001）[2],[3]。值得注意的是，基于mariposa-2临床试验结果，美国国家综合癌症网络《nccn临床实践指南》推荐埃万妥单抗联合化疗作为经典egfr突变晚期nsclc患者接受奥希替尼治疗进展后的唯一1类优选治疗方案[4]。
肺癌在我国所有恶性肿瘤中发病率和死亡率均居首位[5]。据统计，我国每年肺癌的新发病例超过100万例，占全球肺癌患者总数的三分之一以上[6],[7]。egfr基因突变是非小细胞肺癌中最常见的驱动基因，我国约有40%的患者携带这一突变类型[8]。egfr经典突变类型繁多，其中19外显子缺失和21外显子l858r点突变是egfr最常见的突变类型[9]。据统计，接受egfr酪氨酸激酶抑制剂（tki）治疗的egfr突变晚期非小细胞肺患者，其5年生存率低于20%[10],[11]。既往接受egfr tki单药治疗后出现的获得性耐药机制多种多样且呈多克隆性。这使得疾病进展后的靶向治疗更加困难，从而限制了靶向治疗的疗效[12],[13]，而在化疗的基础上联合免疫治疗也未能显示出具有临床意义的改善[14],[15]。因此，对于egfr tki治疗后疾病进展的egfr突变非小细胞肺癌患者来说，埃万妥单抗联合化疗方案的出现为这部分患者提供了一个重要的全新治疗选择。

埃万妥单抗联合化疗的安全谱与既往单药治疗的安全谱一致，因治疗相关不良反应而导致停药的患者比例为10%[1]。
